Dermal fillers

Restylane is the most well known dermal fillers in the United States, known for its safety, efficacy and longstanding results. Restylane is made out of Hyaluronic acid which closely resembles the same substance that occurs naturally in the body. By replenishing the level of Hyaluronic acid in the skin, fillers can immediately soften lines and wrinkles seen with aging (such as the nasolabial folds) and enhance natural features such as the lips or cheeks. Restylane is injected with a very fine needle during a comfortable procedure that requires no downtime. Because Restylane does not contain any animal-derived ingredients, no skin test is required.

Results are immediate and one should expect to leave the office with natural appearing, softer facial contours and a revitalized appearance.

What areas can be treated?

Restylane treatments can be used to smooth out facial lines, create fuller lips(see Lip Augmentation) and shape facial contours, such as cheeks and chin. It is a subtle approach to enhancing looks naturally. The most common uses of Restylane are:

How long does it take to see the results and how long does the Restylane treatment effect last?

The beneficial effects of Restylane are noticed immediately and actually improve over the first weeks as the product absorbs water.

One of the great advantages of RESTYLANE is that the effects are long lasting but not permanent. You are always in control of your looks. Lip enhancement: follow-up treatment is normally needed in about 6 months. Duration depends on many factors, such as skin type, lifestyle, age, the degree of perfection demanded by the patient and the injection technique of the practitioner. Facial lines and facial shaping follow-up treatments are usually needed after 6-9 months whereas lip enhancement results last about 6 months.

Part of what determines the longevity is whether enough volume is used initially to correct the volume defect. Ideal results may require more than one syringe of Restylane.

Do Restylane treatments hurt?

Restylane injections are well tolerated. For patients that desire it, a topical anesthetic can be used. For areas like the lips, local anesthetic injections like at the dentist, are used to numb the treatment area.

Is there any downtime with Restylane?

You may notice slight swelling, redness or bruising following Restylane injections, not unlike any other injection. When they occur, these effects are temporary and don’t generally interfere with any activities. With lip enhancement, the swelling may last a few days so chosen a good time for this treatment is advised. Longer periods of inflammation and swelling are quite rare.

It is advisable to avoid sunbathing, exercise, alcohol and cold outdoor activities for six hours after treatment.

Are there situations where Restylane or other fillers should be avoided?

Restylane should not be used in areas of active skin infection, including herpes outbreaks. Restylane hasn’t been tested in pregnant or breast-feeding women.

What should I do prior to treatment?

For a one week period before injections, It is advisable to avoid taking St. John’s wart, high doses of vitamin E, aspirin, advil, nuprin and any other agents that can increase bruising. If you are prone to cold sores we may want to institute antiviral therapy before injecting the lips.

What would I do in the unlikely event that I wasn’t happy with the results?

The most common reason for disappointment with filler injections is inadequate volume being utilized. It may take more than one syringe of Restylane to fully correct the defect being treated. In the scenario of an undercorrection, further injections will offer additional improvement. In the rare case of overcorrection, time is the best healer. Since Restylane® is gradually absorbed by the body, the extra volume will gradually fade away.

What happens if I discontinue Restylane® treatment?

Medically, there is no reason why Restylane® use cannot be terminated at any time. Recent studies have established the fact that in addition to replenishing lost volume in the skin. Filler injections actually cause collagen synthesis in the body as well.

Can Restylane be used with BOTOX Cosmetic, lasers or with other dermal fillers?

It is very common to combine dermal filler injections such as Restylane with other aesthetic treatments including BOTOX Cosmetic, laser resurfacing or other fillers.

What is the difference between Restylane and Perlane?

Perlane® contains a larger concentration of the active ingredient, hyaluronic acid. Because of this higher concentration of hyaluronic acid, Perlane® is usually used for deeper and larger folds, while Restylane® is better suited for smaller wrinkles or to maintain the results of a previous Perlane® injection.

Juvederm®

Juvederm Ultra™ and Juvederm Ultra Plus (Juvederm) are non-animal derived dermal fillers consisting of a smooth gel that when injected help to instantly restore your skin's volume and smooth away facial wrinkles and folds such as the nasolabial folds—the creases that run from the bottom of the nose to the corners of the mouth). Juvederm is made of Hyaluronic acid, a naturally occurring substance in the body that helps to hydrate and add volume to the skin. The hyaluronic acid in Juvederm supplements the body’s natural supply of this substance.

Filler injections represent the fastest growing non-surgical aesthetic procedures in the United States. What makes Juvederm™ different than other dermal fillers?

Juvederm™ has a high concentration of cross-linked hyaluronic acid that is responsible for the longevity of results. Because the gel is smooth in consistency, this provides for a natural feel in the skin.

How long do the results last?

The FDA recently cleared Juvederm for longevity of one year. Some individuals may find the result begins to wane after about six months and additional treatment would be required. Part of what determines the longevity is whether sufficient volume was used initially to correct the volume defect. Ideal results may require using more than one syringe of Juvederm. For budgetary reasons sometimes only one syringe is utilized initially with the understanding that the results of the first treatment will not last as they would otherwise. To maintain the ideal result it may only take one syringe in the future.

How is Juvederm™ different from BOTOX® Cosmetic?

Juvéderm™ is used to treat wrinkles and folds resulting from a loss of volume beneath the surface of the skin such as in the nasolabial folds. BOTOX® Cosmetic relaxes muscle activity and is used to treat moderate to severe lines caused by repetitive muscle activity. Common areas where BOTOX Cosmetic is used includes the area between the eyebrows (the glabellar area), crows feet region and in the forehead. It is very common for Juvederm and BOTOX Cosmetic to be used in combination for long lasting, exciting results.

Is treatment with Juvederm™ injectable gel painful and is downtime required after treatment?

Juvederm injections are tolerated quite well. Some patient’s desire pre-treatment with topical anesthetic. For lip enhancement we recommend local anesthetic injections similar to what a dentist provides. There is no discomfort after the procedure. If there is mild swelling or bruising, this is generally short lived.

What are the possible side effects of JUVÉDERM™?

Most side effects are mild in nature, and their duration is short lasting (5 days or less). The most common side effects include, but are not limited to, temporary injection site reactions such as: redness, pain/tenderness, firmness, swelling, lumps/bumps, bruising, itching, and discoloration. As with all skin injection procedures there is a risk of infection. Complications are very rare with Juvederm.

What are the pretreatment recommendations?

For a one week period before injections, we suggest avoiding St. John’s wart, high doses of vitamin E, aspirin (unless prescribed by physician), advil, nuprin and other agents that can increase bruising. If you are prone to cold sores we may want to institute antiviral therapy such as Famvir or Valtrex. There is no pre-treatment allergy skin test required before the use of Juvederm.

Can people with different skin tones/colors be treated with JUVÉDERM™?

Yes. In fact, Juvéderm™ injectable gel is the first FDA-approved hyaluronic acid dermal filler that has proven its safety and effectiveness in persons of color. Studies with Juvéderm™ showed no increased risk of hyperpigmentation or hypertrophic scarring in patients of color.

Perlane®

Perlane® is injectable cosmetic dermal filler that has been used to soften wrinkles and facial folds of patients across the world. It is also popular filler for lip augmentation, facial depressions, and facial scarring. It has been used worldwide since 2000 and received FDA clearance in 2007.

Perlane® is made from a non-animal derived hyaluronic acid gel. Since hyaluronic acid is a natural substance found in the skin and other tissue, there is no need for a skin test when using Perlane. The hyaluronic acid in Perlane® supplements the body’s natural supply, which becomes depleted with age. Perlane® is biodegradable so its effects are not permanent. The results of a Perlane® filler treatment tend to last six months to a year. This is much longer than the typical results of collagen injections which may only last three to six months.

What Areas of the Face Can Be Treated with Perlane®?

The hyaluronic acid in Perlane® can be used to improve the contour of facial skin and to reduce wrinkles, folds, scars, and other facial skin depressions. It is used in both sexes.

Are treatments painful and what can I expect afterwards? Are there side effects? Anything special to know beforehand?

Perlane injections are provided thru ultra-fine needles and the process is well tolerated Most treatments take about 20-30 minutes to complete and can be done without any anesthetic, but a topical anesthetic cream can be utilized beforehand if requested. Patients may notice mild swelling or bruising after the treatment and these effects will subside on their own. You should avoid exposure the treated areas to excessive sun or UV lamps or extreme cold until any redness or swelling has resolved. Complications from Perlane are rare. It is recommended to avoid taking aspirin (unless medically indicated), ibuprofen, St. John’s wort and high doses of vitamin E for a week before treatment to decrease the risk of bruising at the site of injections. The injection process also tends to be a bit more uncomfortable, though the application of a topical anesthetic to the skin beforehand helps greatly. Radiesse cannot be used in the lips or in the frown area.

Is Radiesse permanent?

Treatment results are relatively long-lasting, but not permanent. Over time the tiny calcium-based microspheres gradually break down into basic calcium and phosphate ions which are safely and naturally absorbed into your body, leaving your own collagen in their place.

How is a Radiesse treatment performed? Is downtime required?

After a complete evaluation and discussion, pre-treatment photos are obtained and the skin is numbed with a topical anesthetic. Radiesse is then injected into the skin using ultra-fine needles during a procedure that takes approximately 20-30 minutes. We apply some ice at the office and you leave ready to resume your day. Little or no downtime is expected after Radiesse injections. It is suggested to avoid intense sun exposure, extremes of cold and exercise for 24 hours. If any lumpiness is detected after 2-3 days, it is recommended that you return to the office for evaluation.

Could I have an allergic reaction or other side effects from Radiesse?

Radiesse is biocompatible, meaning there is virtually no risk of an allergic reaction. You do not need to undergo skin allergy testing, common for certain animal-based or permanent filler agents. Normal, injection-related reactions such as mild irritation, edema, swelling, itching, discoloration, or tenderness at the injection site may occur. These conditions typically resolve on their own within one to two days.
